COBRA
This option allows you to temporarily continue the coverage you had through your previous employer (typically for up to 18 months). Most companies with 20 or more employees have to offer COBRA. There are strict deadlines to enroll, usually within 60 days of the end of your employment. To find out if you qualify for this coverage, please refer to the materials sent to you by your previous employer or talk to your previous employer right away.
Cal-COBRA
This option allows you to temporarily continue the coverage you had through your previous employer. Most companies with less than 20 employees have to offer Cal-COBRA. There are strict deadlines to enroll, usually within 60 days of the end of your employment. You may also be eligible if you were enrolled in COBRA coverage for 18 months or until it expired. Kaiser Permanente Conversion Plans
We offer these plans to people who have lost or discontinued their coverage. These plans don't require a medical review. You must enroll within 63 days from the date of departure from your previous plan—and there will be no lapse in coverage. To find out if you qualify for a Conversion Plan, please call Kaiser Permanente Member Services at 1-800-464-4000.
Kaiser Permanente for Individuals and Families
We offer a variety of copayment and deductible plans to choose from depending on your needs and budget. These coverage options include a number of different health care benefits, services, and resources. A medical review is required for applicants over the age of 18. MRMIP (Major Risk Medical Insurance Program)
This program is partially paid for by the state of California, and specifically designed for people who don’t qualify for individual plans because of pre-existing conditions.
To enroll, you must apply through the state of California. There may be a waiting list.

Medi-Cal
This government plan is California’s Medicaid public health insurance program. It provides health coverage for low-income and/or disabled individuals and families. It includes inpatient and outpatient health as well as prescription drug coverage. It also covers a variety of pre-existing conditions and special needs. Medicare with Medi-Cal Part A/B
This state-funded plan is designed for people with a disability who are eligible for Medicare. It includes inpatient and outpatient health coverage. To find out if you qualify for Medi-Cal Part A/B, please call 1-800-952-5253 or 1-888-747-1222, or visit www.medi-cal.ca.gov.
Kaiser Permanente Child Health Plan (KPCHP)
This plan is designed for children up to age 19 from families with limited income who do not qualify for Medi-Cal or the Healthy Families Program. In addition to health care benefits, this plan also includes vision and prescription drug coverage. Enrollment is limited, so there may be a waiting list. AIM (Access for Infants & Mothers)
This government plan provides comprehensive medical care to expecting mothers—and is effective for up to 60 days after your infant has been delivered. After birth, your infant will be eligible to be enrolled in a Healthy Families Program until his or her first birthday. There are income requirements to enroll in AIM. Healthy Families Program
This program is also known as the Children’s Health Insurance Program (CHIP). It provides comprehensive coverage for children who do not have employer-sponsored insurance and who do not qualify for no-cost Medi-Cal. Kaiser Permanente Senior Advantage (HMO) or Medicare Plus (Cost)
Our Medicare health plan is called Senior Advantage or Medicare Plus, depending on your area. Both plans give you all of the benefits of a Kaiser Permanente membership along with Medicare Part A hospital care, Part B medical services, and Part D prescription drug coverage—all in one plan. Alternate Group Coverage
This coverage is provided by the employer or organization of a person in your household, such as a spouse, domestic partner, or parent (for children up to the age of 26). Different employers have different rules about who can enroll. To find out if you qualify, please check with the employer for eligibility information.
PCIP (Pre-Existing Condition Insurance Plan)
This government plan is designed to help people with pre-existing conditions. It includes a variety of benefits, such as primary and specialty care, along with hospital care, and prescription drug coverage. To enroll, you must apply through the state of California.
